WOOD CANYON ELEMENTARY

Welcome to Wood Canyon Elementary School. To begin the registration process for your child, please go to the school’s website at https://woodcanyon.capousd.org and begin the on-line process via the Enrollment tab. Please be careful to select the correct year of enrollment.

 

·         A valid address in the Wood Canyon Elementary School

·         A valid e-mail address

·         General information about your student

·         Name, address and district of previous school, if applicable

·         Parent/Guardian’s home/work phone and address

·         Name and phone number of emergency contacts (at least 3 emergency contacts)

·         Immunization dates

·         Local physician name and phone number

·         If the student was not born in the USA, you will need the date the student entered the USA

 

Once the online registration is complete, please gather the required documentation (for TK/Kinder enrollment, complete the packet distributed at the TK/Kinder Roundup meeting). Please bring the following required documentation to the school office Monday - Thursday only, from 9am - 1pm.

 

      ·         Print and sign the On-line Registration Confirmation form

      ·         Child’s birth certificate (Original)

      ·         Immunization record (Original)

      ·         Two proofs of residency (current utility bill i.e. gas, water, or electric and a mortgage statement or rental agreement)

 

      ·         Oral Health Assessment Form - Completed and signed by dentist (Kinder only) 

      ·         Wood Canyon Elementary Enrollment Information form

      ·         Indian Education 506 form - only if applicable

      ·         Home Language Survey form
